We have an exciting opportunity for an NDIS Support Coordinator to join our existing vibrant, dedicated & growing team.

At Mentis Assist we support people to live a full and meaningful life. Working alongside and empowering those with mental health concerns is why we are here. We have been partnering with people with a mental health diagnosis across the Mornington Peninsula and Frankston region for over 30 years and have expanded our services into the Casey region. We offer a range of supports to assist people to recognise their values, strengths and goals, with an emphasis on improving health and wellbeing, independence, social participation and enhancing meaningful relationships with family and significant others.

The NDIS supports people with a persistent disability that affects their ability to take part in everyday life, and gives people choice and control in how, when and where the supports in their plan are implemented.  The Support Coordinators work with a case load of NDIS clients to commence and implement the necessary supports within their unique NDIS plan.

Our NDIS teams’ partner with participants with NDIS plans to work collaboratively and achieve their NDIS goals.

Our team of support coordinators and recovery coaches are passionate about creating lasting changes for our NDIS participants. We work closely to encourage and learn from each other, and to stay up to date with the sector changes. Mentis Assist has a supportive workplace culture, with dedication to ongoing staff professional development and wellbeing.  Staff are provided with use of pool vehicles, a mobile phone, and a laptop for work use.

We are looking for someone who has:

  • A strong understanding of the needs of people living with psychosocial disability.
  • Strong case management and engagement skills
  • The ability to collaborate and negotiate with multiple stakeholders carers
  • A person centred and strengths-based approach
  • The ability to think innovatively and creatively to help clients achieve their NDIS plan goals
  • Excellent time management and administrative skills

We highly regard:

  • Qualifications in community services, mental health, social work, nursing, or other allied health practice
  • Previous NDIS experience or understanding
