Based on the Mornington Peninsula, with offices in Mornington, Frankston, Casey and Cardinia, Mentis Assist provides support for people with complex Mental Health issues.

Join us as our HACC- PYP Linkages Packages Program Case Manager. We are looking for someone that shares the values of Mentis Assist; challenging stigma, thinking outside the box, working with an awesome team producing positive outcomes for your clients and job satisfaction for yourself.

The HACC – Program for Young People is a person centred program, supporting people under the age of 65 or Aboriginal or Torres Strait Islander people under the age of 50 with complex care needs who would benefit from case management to help coordinate their health care needs. The program seeks to build client’s capacity and independence, while respecting their diversity of culture and needs.

Successful applicants will have tertiary qualifications (minimum Cert 4 level) in mental health, community services, nursing and a minimum of 2 years’ experience working in the areas of mental health, HACC, case management or similar. Your ability to manage a case load of clients, oversee brokerage budgets, individual packages and service coordination is essential.

Working 4 days per week, based in Mornington this position is classified at SCHADS Level 4.
