The North and West Metropolitan Region Palliative Care Consortium is an alliance of organisations providing palliative care for people living in the north and west metropolitan region of Melbourne. Consortium members include community palliative care services, public hospitals (palliative care units and palliative care consultancy services) and the North Western Melbourne PHN.
Our Consortium:
Melbourne City Mission (MCM) is the fund holder and employing agency for the Consortium. The roles and services of the Consortium and MCM function separately, with the Consortium Management Group being the key driver of Consortium priorities.
Our Consortium is seeking a Registered Nurse (Division 1) or Allied Health clinician with project management experience to work part time (0.8 EFT) within a small, dynamic and supportive team to continue the implementation of our Palliative Care in Aged Care and Disability Strategies.
This is a 12-month role with the possibility of an extension beyond June 2023.
Continue the implementation of the Palliative Care in Aged Care Strategy:
Implementing the Palliative Care in the Disability Sector Strategy. Key responsibilities include:
Evaluating Strategies by working with the Consortium Manager to define performance measures for strategic objectives and collect and report outcomes date and information to the Consortium Manager and Consortium Management Group and undertaking report writing and other project related tasks as required to fulfil obligations of the project.
