With head office based in Maribyrnong, and frontline staff working throughout the northern and western region of Melbourne, MCWA has/is supporting 000's of ageing and elderly members of the community.

About the Role

We currently have an exciting opportunity for an experienced and committed Case Manager that is looking for their next career move. MCWA seeks to appoint a head of Home Care Packages to lead and work collaboratively with the MCWA Home Care Package team.

As part of your duties, the Home Care Packages Manager will be tasked with acquiring and managing the Home Care Package clients, and working with the team to manage client needs.

Position is available for 4 days a week (salary is pro rata).

Main duties/responsibilities

  • Report directly to the Managing Director
  • Acquire and manage new Home care packages clients
  • Lead a team of Case Managers
  • Work with internal stakeholders to ensure clients needs are being met
  • Provide high level comprehensive case management for individual package clients ensuring there is encouragement for both the clients and the carers to engage in the planning and the implementation process
  • Provide high level client experience, maintain a client-centered approach
  • Recognise, anticipate and focus on specific client needs and advocate where required
  • Work with the team to develop, review and reassess client care plans and goals together with clients and support network
  • Work with the team Establish, manage and ensure services are operating within the approved individual budget
  • Work with the Finance team to reconcile Medicare statements & Home care manager
  • Establish, coordinate and manage third party services
  • Lead, manage and inspire the Home Care Package team
  • Maintain current knowledge of the Home care package service guidelines and changes
  • Prepare reporting as per the funding agreements

Key selection criteria:

  • Tertiary qualifications and/or related field experience
  • Demonstrated understanding of the Aged Care Quality Standards
  • Demonstrated understanding of the needs of older people, people with disabilities and their carers
  • At least 2 years experience in Case Management of Home Care Packages
  • Demonstrated ability to create and implement consumer-directed services, with a particular focus on working with CALD consumers
  • Demonstrated experience in budgeting and financial accountability reporting.
  • Demonstrated ability to manage conflicts and complaints
  • Ability to speak and understand Macedonian language
  • A current Driver's License
