MACS Wellbeing Co-ordinator/Care Manager is a full time position and is responsible for managing the day-to-day service provision to MACS’ home based consumers. Each MACS’ Home Services Wellbeing Co-Ordinator has an allocated caseload, but may also be required to contribute to overall consumer management. Duties include, but are not limited to, consumer assessment, community and home care staff liaison, and answering daily queries and problem solving with Consumers and families.
