About the opportunity

As a Case Manager - Pre Residential and Post Treatment you will deliver individual and group-based counselling and support services to proactively assist and support clients with entry and exit into our residential treatment programs in a way that maximises person-centred care in an integrated, supportive, and professional manner. This role is based in Burleigh Heads.

Key elements of the role include:

  • Work with the residential treatment teams and clients for entry to the residential facility
  • Provide recommendation for entry to residential programs based on the results of assessment, screening and risk assessments
  • Work closely with the Clinical Services Manager to maintain and grow the occupancy of the service, managing enquiries, assessments, admissions, and client demand
  • Play an active role in the development of transition plans to support effective person-centred service delivery, particularly when supporting admission and discharge of clients from residential treatment
  • Ensure continuity of care by effectively transitioning clients in and out of residential treatment utilising skills and expertise in multidisciplinary teams and effectively negotiating and advocating for client support needs with other health care providers

What you will bring to the role

  • Tertiary qualifications in a relevant human services discipline with additional AOD qualifications highly regarded.
  • Applied knowledge in contemporary evidence based AOD counselling practice, case management, outreach, and intake and assessment.
  • Demonstrated experience in facilitating groups
  • Demonstrated experience in working autonomously – especially in terms of managing competing work demands and your own work schedule
  • Experience in a role where you have successfully communicated across various work teams
  • Experience in a phone-based contact role.

This is a max-term contract until 30 June 2022 with possible extensions.

The salary $84,849.44 per annum. We are fortunate that our employees can also access up to $18,540 per annum in salary packaging which increases their take home pay.
