ABOUT THE ROLE

This role’s purpose is to support up to five young people to gain the skills needed to transition into meaningful independent living. This is achieved through forming connections with young people within a spacious six-bedroom home. The primary duties will involve a combination of care planning, delivery of individualised and community therapeutic interventions, and supporting the young people with involvement and participation in the home. The role will be primarily based from a Lighthouse home and will require working one day a week working as a team at our Youth Resource Centre in Richmond. You will be highly valued member of the team, supported with fortnightly clinical and operational supervision.

Your responsibilities will be, but are not limited to the following:

• Forming attachments with young people
• Empowering young people to succeed and have a voice
• Creating and implementing goals with young people
• Liaising and networking with external professionals
• Monitoring consistency of operations in the home and managing day-to-day maintenance and safety requirements
• Attending young people’s Care Team Meetings when appropriate
• Ensuring care is administered in accordance with the Lighthouse Model of Care
• Overseeing and delivering therapeutic Lighthouse interventions in collaboration with the Care Team
• Modelling Lighthouse values and culture within the home
• Working from the home and meeting with young people regularly to ensure high quality of care
• Ensuring all appropriate processes are followed regarding critical incidents and reporting requirements
• Maintaining effective case noting and other forms of internal and external reporting

ESSENTIAL REQUIREMENTS

• Qualifications in a related discipline (e.g. social sciences, health, psychology, social/youth work, community services, management)
• Experience in a care-oriented role
• Demonstrated capacity to understand and work with young people, and knowledge of the unique needs of young people who are experiencing or at risk of homelessness
• Demonstrated ability to work effectively within a collaborative team approach to service planning and program implementation
• Ability to plan, prioritise workloads and meet deadlines as required
• Experience with conflict resolution and mediation
• Excellent written and verbal communication skills
• Hold/obtain a current Working with Children Check
• Have Working rights within Australia and have evidence to confirm this
• Complete a National Criminal Records check, Disqualified Carer check, and undergo an in-house psycho-social screen
