About the Role

We are experiencing exciting growth within our Aged Care Program in the ACT,  and are now looking for multiple Team Leaders for our service delivery teams based in Canberra. We are currently seeking to recruit;

  • 1 x Permanent Full time position
  • 1 x Full Time, maximum term contract (to 26 August 2022)

We provide community based home care support to a large number of clients across Canberra and surrounds, and this role will be focused on services within our Home Care packages program

The ordinary hours of work are Monday - Friday during business hours, however, Some work outside of core hours may be required to accommodate workplace flexibility to ensure the needs of clients and staff members are met.

We are looking for someone with prior leadership/coordination experience within the Aged Care industry and a strong understanding of the Aged Care Standards and Home Care packages.

Key Responsibilities

  • Develop care plans in partnership with consumers and their representatives to set and meet goals and achieve outcomes.
  • Coordinate and monitor high quality care and support services
  • Monitor Home Care Package individual costs, ensuring services are within budget constraints, guidelines, and Home Care Quality Standards.
  • Monthly data entry for reporting requirements.
  • Develop, lead, motivate and manage a small cohort of community-based support workers
  • Liaise with internal and external stakeholders.
  • Model appropriate conflict resolution, communication, and positive behaviour management skills, point of escalation and adjudication where relevant for client and direct staff feedback
  • Communicate and act in ways that are consistent with Life Without Barriers values: Responsive, Imaginative, Courageous, Respectful and Relationships

Skills & Experience

  • An unrestricted Australian Driver’s Licence
  • Demonstrated experience of working in a leadership or coordination role within the Care Industry
  • Demonstrated knowledge of person-centred thinking, Aged Care Standards and an understanding of Aged Care Home Care Packages

Successful candidates will be required to clear relevant state-based probity checks including National Criminal History Record Check and the ACT Working With Vulnerable People Card.
