About the role

The Team Leader demonstrates organisational and leadership skills in the coordination of aged care services to assist our clients. The position is focused upon ensuring that the individual needs of clients are met, whilst maintaining an efficient and safe working environment in our Aged Care Program.

Key Responsibilities

• Coordinate and case manage the delivery of Aged Care services
• Manage referrals coming through from My Aged Care and signing up clients
• Apply the relevant aspects of the aged care legislation, and the Commonwealth Aged Care program guidelines;
• Ensure services are delivered in accordance with the Aged Care Quality Standards and Quality Review processes and funding agreements
• Excellent interpersonal skills and high-level written and verbal communication skills
• Provide supervision, rostering, training and support to direct reports

Skills & Experience

• Diploma in relevant Social & Community Services qualification
• Demonstrated experience of working in a leadership role in a human services organisation
• Strong coordination, leadership and peer support skills
• Established problem solving and conflict management skills
• Demonstrated experience in maintaining and providing service user and staff confidentiality
