About the role

Working within our Aged Care Services function, you will be responsible for overseeing and managing a small, dedicated team of Client Support Officers. You will be instrumental in the effective delivery of support services to ageing clients, clients with complex health needs and vulnerable people requiring support in the community. 

This is a Permanent Full-Time position based in Canberra. Whilst it is anticipated that the days of work for this role will be Monday – Friday, please note that flexibility to work after hours and on weekends may be required in order to meet client support needs.  

Key Responsibilities

  • Coordinate and manage a small team of Client Support Officers
  • Provide ongoing guidance, mentoring and lead from the front
  • Establish and maintain positive relationships with stakeholders including clients, families and guardians
  • Assess, develop and regularly review client care plans, service agreements and available client documentation to ensure services are being delivered as agreed with client and/or their authorised delegate
  • Undertake regular reporting and documentation for both internal and external stakeholders
  • Provide direct support, care and assistance to clients as required to meet their support needs

Skills & Experience

  • Certificate IV Community Services, Individual Support, Aged Care (or similar)
  • Extensive knowledge and experience working with Home Care Packages
  • Previous experience in leading and/or mentoring a team utilising positive and proactive approaches. Prior clinical care experience within aged care, community, or health would be highly advantageous.
  • Demonstrated experience supporting clients who are ageing and/or have complex health needs
  • High level written and verbal communication including reporting, stakeholder liaison and negotiation skills 
  • Current Drivers Licence & First Aid certificate

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working With Vulnerable People Card (where relevant to the role).
