Extended Transition Support is a program directed at supporting people in the community with severe and persistent mental illness and complex needs. The program will support people to test their eligibility for supports under the NDIS. Clients will be supported to transition to the NDIS if deemed eligible or Continuity of Supports if not eligible. The program also assists and coordinates multiple services and support systems in an integrated way so that people with severe and persistent mental illness and complex needs are better supported to access the right service at the right time in their journey to recovery.
We are currently seeking a suitably skilled and enthusiastic individual to join our team as a Support Facilitator on a part-time (30 hours per week) basis until 30 June 2020 located in Christies Beach.
The role is suited to an experienced professional with a background in working with clients with complex needs and mental health issues, an understanding of the NDIS, a commitment to better outcomes for this target group and for those with a passion and aptitude for strong consumer advocacy.
As a Support Facilitator you will work with clients applying a person-centred, recovery focused approach. Highly developed interpersonal communication skills will be required, including relationship building, negotiation, advocacy and conflict resolution. Experience working in a culturally sensitive manner with Aboriginal and Torres Strait Islander people and a willingness to work and train in culturally appropriate approaches; Current Driver Licence.
Successful candidates have a minimum Certificate IV in Mental Health and will be required to clear probity checks including a DCSI Child Related employment screening and in-house National Criminal History Record Check.
