About the role

The purpose of the Support Coordinator, Disability is to assist NDIS participants in the support coordination and implementation of all supports in their funded plan, including informal, mainstream and community supports, as well as funded supports. The successful applicant will have strong communication and computer skills, and the ability to think creatively and use initiative, work independently and as part of a multidisciplinary team using strong coordination, leadership and peer support skills. We are looking to fill this position on a full time basis, for a maximum term period of 12 months, parental leave cover. The role will be based in our office at Hobart with work from home opportunities also available.

Key Responsibilities

  • Support the customer to implement their NDIS plan, including assistance to identify and select providers
  • Monitoring plan outcomes & plan expenditure to ensure the plan remains within budget
  • Strengthen and enhance the customer’s abilities to self-direct their own supports and participate in the community;
  • Ensure mainstream services meet their obligations (i.e. housing, education, justice, health)
  • Ensure clear, accurate and timely record keeping including making information accessible to the customer
  • Capture any required data for the organisation and the Agency

Skills & Experience

  • Relevant Tertiary qualifications including Diploma in Disability; or Certificate III or IV in Disability or equivalent
  • Demonstrated experience in working with people with a disability, including supporting people with disability to develop positive relationships with a broad range of internal and external stakeholders
  • Demonstrated experience in facilitating personal planning processes
  • Demonstrated application of the objectives and principles of the National Standards for Disability Services
  • A professional and positive outlook about supporting people with disabilities and commitment to supporting and promoting the individual needs, skills, abilities and personal goals of people with disabilities.
  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the National Disability Insurance Scheme

Successful candidates will be required to clear probity checks including National Criminal History Record Check and hold Tasmanian Working With Vulnerable People Registration with NDIS Worker Screening.
