The purpose of the Support Coordinator, Disability is to assist NDIS participants in the support coordination and implementation of all supports in their funded plan, including informal, mainstream and community supports, as well as funded supports. The successful applicant will have strong communication and computer skills, and the ability to think creatively and use initiative, work independently and as part of a multidisciplinary team using strong coordination, leadership and peer support skills. We are looking to fill this position on a flexible permanent part time or full time basis for the right candidate. The role will be based in our vibrant office at Hobart with work from home opportunities also available.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and hold Tasmanian Working With Vulnerable People Registration with NDIS Worker Screening.
