About the role

Based in Tuggerah on a 6 month contract, this role will assist NDIS participants in the coordination and implementation of all supports within their funded plan. You will also establish strong rapport and working relationships with a range of stakeholders including People with Disability, their family members and internal & external service providers.

Key Responsibilities 

  • Support LWB customers in the implementation of their NDIS plan
  • Monitor plan outcomes & expenditure to ensure the plan remains in budget
  • Ensure mainstream services meet the customer’s obligations (housing, education, justice and health)
  • Provide LWB customers and the NDIA with reports on outcomes and success indicators
  • Build the capacity of the LWB customer to achieve greater independence to self-direct services and supports in the longer term

Skills & Experience

  • Minimum Certificate III or higher in Disability or Mental health or equivalent
  • Demonstrated experience in working with people with a disability including supporting them to develop positive relationships with stakeholders
  • Ability to think creatively, use initiative and problem solve both independently and as part of a wider team
  • Current NSW driver's licence
