A new opportunity has opened up in the Support Co-ordination team to support our clients to bring their NDIS plan to life by assisting them to co-ordinate their supports both through their NDIS plan and other community or mainstream services.

Support Co-ordinators play a critical role in supporting participants to navigate the NDIS system, understand their NDIS plans, connect into the broader system of services and to build their capacity to direct their supports independently over time.

This is a service which is rapidly growing in the NDIS sector and within LWB and we are excited about being able to expand our reach to help more clients.

About the Role

This role will form an integral part of the Support Co-ordination team, assisting NDIS participants in the coordination of their supports both through their NDIS plan and community or mainstream services.

Based at our Unley office on Greenhill Road, you will work with the broader Support Co-ordination team supporting clients across Adelaide and may be required to travel to other locations.

Key Responsibilities

  • Research both NDIS funded and unfunded services and supports which will enable our clients to achieve their goals within their NDIS plan 
  • Regularly monitor plan outcomes and expenditure to ensure the services remain within the plan budget
  • Strengthen and enhance our client's ability to coordinate supports and self-direct their services in the longer term
  • Build or support the capacity of clients to develop relationships and partnerships in local communities
  • Ensure mainstream services (i.e. housing, education, justice, health) and NDIS paid supports meet their service obligations to our clients
  • Provide our clients and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency 
  • Clear and accurate record keeping of information

Skills & Experience

  • Demonstrated experience in working with people with a disability
  • Experience in facilitating personal planning processes
  • Excellent communication skills and a demonstrated ability to engage clients and their families
  • Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents
  • Established time management, problem solving and conflict resolution skills
  • Ability to think creatively and use initiative, to work independently and as part of a multidisciplinary team using strong coordination, leadership and peer support skills
  • Minimum Certificate III or higher in Disability or Mental health and/or equivalent experience

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Disability Services Employment Screening.
