About the role

This is a challenging and rewarding opportunity for you to make a positive change in the lives of children and young people in Out of Home Care in Perth Metro. The Senior Case Worker will work alongside the Manager to support the team to meet the safety, wellbeing and goals of children and young people in family based and residential out of home care. The incumbent provides practice mentoring and coaching to Care Team staff to support the continuous improvement of service delivery. Your role will be instrumental in supporting carers to provide a safe and nurturing environment that promotes the physical and emotional safety of children and young people.

This is a full-time permanent position, based from our Leederville office and working from home. This role come with a Tool-Of-Trade vehicle, laptop and mobile phone.

Key Responsibilities

  • Work as part of Care Team to address the impacts of abuse, neglect, separation from families and complex trauma on children and young people in out of home care.
  • Ensure the CARE therapeutic framework guides assessment, planning, implementation, monitoring and review of children and young people’s individual case plans.
  • Participate in and where required coordinate and facilitate Care Team meetings, Reflective Practice, reviews and assessments of young people’s needs as part of the Care Team.
  • Provide practice mentoring to Care Team to model the required work environment, culture, systems and processes necessary to embed standards, behaviours and practices consistent with the values, work health and safety, policy & legislative requirements and pillars of practice.
  • Coach, support, motivate and develop Care Team in best practice.
  • Participate in project work to contribute to continuous improvement of service delivery in line with LWB Strategy and operational plans and relevant state based requirements
  • Attend and as required, represent the Manager at internal and external meetings, maintaining and developing positive working relationships.
  • Develop positive developmental relationships that build trust with children and young people through visible interest and active engagement, and place the best interests of the child at the centre of case planning.
  • Support children and young people to develop and maintain positive relationships with family and community members who can support them while in care and as they transition to independence.
  • Support carers to develop positive developmental relationships with children and young people they care for and to apply the principles of the CARE model to all interactions with children and young people
  • Develop high quality case plans which respond to the individual and changing needs of children and young people in LWB’s care in a timely manner.
  • Develop comprehensive Leaving Care plans for each young person 15 years and over to support their transition to independent living and ensure young people are supported to access financial and other support.
  • Collaborate with Cultural Support Coordinators to incorporate and implement strategies within case plans to support Aboriginal and Torres Strait Islander children and young people to maintain their culture and identity.

*Must participate in on-call roster as required*

Skills & Experience

  • Relevant Bachelor’s Degree and/or relevant experience. The preferred minimum qualifications are Bachelor of Social Work, Social Welfare, Psychology, Nursing and Mental Health. (Note: A qualification is desirable but not essential for Aboriginal staff with appropriate experience and willingness to undertake further training)
  • Demonstrated experience in case planning with children and young people who have experienced trauma with a focus on achieving permanency outcomes
  • Experience developing quality case plans or individual plans 
  • Demonstrated experience working autonomously with multiple clients simultaneously
  • Demonstrated coaching and mentoring skills and experience
  • Current unrestricted driver licence

Successful candidates will be required to clear probity checks including National Criminal History Record Check, Department of Communities Check and Working with Children Check.
