About the role

The Next Step Plus Senior Case Manager will provide a localised collaborative approach engaging and supporting young people from the age of 15, to successfully transition from care; to develop the skills and knowledge they need to live as an interdependent adult; and ensure those who have left care are able to receive additional support and services if needed until they are 25.

This role will also be responsible for providing mentoring and coaching to case management staff to support the continuous improvement of service delivery. This position is being offered as permanent full-time and can be based out of Ipswich or Toowoomba.   

Key Responsibilities

  • Lead and support team members to achieve the business plan outcomes and other organisational programs and initiatives, whilst demonstrating LWB’s values.
  • Monitor, coach, support, motivate and develop team members to enable them to deliver to the best of their ability.
  • Facilitate access to holistic, tailored supports addressing underlying issues which may obstruct the young person’s success in achieving goals.
  • Develop comprehensive plans for each young person to support their transition to independent living and ensure young people are engaged with community supports.
  • Utilise Therapeutic Crisis Intervention techniques to assist in the recognition and de-escalation of behaviour people and help Young People develop ways to cope with stress.
  • Work with Child Safety staff to support young people in care aged 15+ in the development of a transition plan and gaining the skills, knowledge, and networks they need including identifying a natural mentor. 

Skills & Experience

  • Relevant Tertiary qualifications (3-year bachelor’s degree) within the community service sector; Social Science, Social Work, Psychology, Human Services and/or relevant experience.  
  • A current QLD Driver’s Licence.
  • Demonstrated experience in leading and managing teams or individuals.
  • Demonstrated experience in person centred case management and/or coaching/mentoring service delivery and support for young people with complex needs.
  • Demonstrated experience and understanding of the factors impacting on young people in the transition from out of home care to independent adult life.
  • Working knowledge of relevant legislative and regulatory environment.
  • High level written communication skills for the creation of reports, correspondence, and other material.
  • Highly developed analytical skills and the ability to apply these skills in case management/coordination.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
