About the role

Our Aged and Community Care program in Rockhampton is currently looking for an experienced and motivated Program Coordinator to lead a team of Support Workers. This position is being offered on a permanent full-time basis. In additional to an attractive remuneration package, the successful incumbent for this position will also be offered a monthly RDO.

To be successful in this position and dependent upon the arrangements for individual clients you may also work directly with clients from time to time as part of the role as well as participating in on call responsibilities on a rotating basis.

Key Responsibilities

  • Coordinate, develop and lead the Aged and Community Care Program
  • Manage, maintain and adjust client rosters, experience in the TRACCS data system would be an advantage.
  • Work closely with the Program Coordinator and Program Manager, to ensure a consistent standard of quality practice
  • Complete reports and other administration duties as required
  • Provide supervision to your team/s, facilitate Team Meetings on a regular basis, facilitate and arrange required training
  • Develop and maintain functional and supportive relationships with clients in order to promote positive outcomes.
  • Develop and maintain relationships with all stakeholders, including clients’ families and Guardians.
  • Manage communication with key stakeholders, internally and externally
  • Manage, mentor and supervise Program Assistants

Skills & Experience

Ideally, you will have proven Aged Care experience with providing services to clients with Home Care Packages, Commonwealth Home Support Program, Queensland Community Care Services and Veteran’s Home Care.  

  • Tertiary qualifications in the disability or aged care field and/or relevant experience.
  • Demonstrated experience in the Aged Care or health related fields with significant coordination experience.
  • Demonstrated experience of working in a leadership/management role in a human services organisation.
  • Demonstrated ability to work with individuals, families and/or groups from a strength-based culturally appropriate perspective.
  • Ability to develop and maintain positive relationships with a broad range of internal and external stakeholders.
  • Sound project management experience including strategic analytical thinking, planning and delivering.
  • High level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills. 
  • A current unrestricted Driver Licence.
  • Demonstrated capacity to work in a fast-paced environment dealing with complex and challenging situations.
  • Valid Yellow Card / NDIS Workers Check or willingness to obtain one at your own cost
  • Valid Working with Children's Check or willingness to obtain one at your own cost

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
