About the Role

We are looking for someone to join our Supported Independent Living team supporting one male client who lives in Orelia. Our client has a complex mental health diagnosis so we're looking for a Disability Support Worker with mental health experience.

As a Disability Support Worker, you will play an important role in supporting and empowering people with disability to live independently, access the community and to achieve their individual goals. Working in a team, you will be actively supporting individual needs and creating a safe environment.

This permanent contract is for sleepover shifts and the roster (although subject to change) is as follows:

Sleepover Roster:

  • Start time 17:00 (Sleep 22:00-06:00) Finish Time 09:00
  • Week 1 - Tuesday, Wednesday, Thursday, 
  • Week 2 -  Monday, Friday, Saturday, Sunday

Key Responsibilities

  • Support in day to day living, promoting cooking, housekeeping, hygiene, shopping etc.
  • Support clients to work towards achieving their individual aims
  • Liaise and collaborate with clients, their community and community support agencies
  • Monitor and review the client’s progress through the service and against the case plan
  • Maintenance of progress notes and record-keeping

Skills & Experience

  • Experience working with clients with mental illness, complex needs and behaviours
  • Certificate III Individual Support, Disability or similar
  • Good computer literacy and administration skills to complete progress notes and use online IT systems
  • Unrestricted Australian Drivers Licence
  • Current Provide First Aid Certificate HLTAID003 & HLTAID001 (mandatory prior to offer)
  • NDIS Worker Orientation Module certificate
