About the role

This role is responsible for providing support and information for people with disabilities, across their life-cycle and in all domains of life, to enable access to a quality life with dignity, respect and social inclusion in the least restrictive environment. 

We are looking for an experienced Disability Support Worker (DDSO2) to join our team in St Kilda in this rewarding Part -Time opportunity. You will be responsible for providing support with activities of daily living and developing, implementing and monitoring Individual Plan’s including personal care. This role provide a career path into potential House Supervisor opportunities, in a supportive, mentoring environment.

In this role you will work to facilitate the delivery of quality services and achieve positive outcomes with and for the 5 clients within the house. This role involves working with clients with various complex behaviours and includes sleepovers.

Key Responsibilities

  • Provide practice support and supervision to other team members
  • Directly support people with disabilities, within their home and community, to participate in a range of life areas (including social, recreational and daily living activities) based on the goals of individual support plans
  • Advocate for people with a disability to improve client outcomes
  • Administer medication treatments and therapies
  • Conduct interviews with people living with a disability, and their family
  • Develop and maintain client support plans (including health specific or positive behaviour plans for people with complex needs)
  • Provide information, advice and referral, where required, to crisis intervention services for people with a disability, their families, carers and/or service providers
  • Manage a caseload that comprises people with a disability, their families and carers (under professional supervision)
  • Develop, implement, monitor and review client support plans (under professional supervision)
  • Coordinate and negotiate with family members, health services, activity providers, and other community services to help meet daily client needs and/or to plan support for future client requirements
  • Under general direction, undertake more complex direct care functions or contribute to the design and implementation of a range of plans where appropriate

Skills & Experience

  • Certificate IV in Disability or equivalent Disability related qualification
  • Current Employee Working with Children Check (WWCC)
  • Level 2 First Aid Certificate
  • Full driver’s licence
  • Interpersonal skills: Ability to see things from other’s point of view and able to express your own views in a constructive and diplomatic way
  • Advanced computer skills
  • Strong Stakeholder management skills
  • Self-management: Able to accept responsibility for own actions and has a realistic and balanced view of own strengths and weaknesses

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
