About the role 

As a Case Manager for our Intensive Therapeutic Care – Significant Disability (ITC-SD) program, you will develop and implement therapeutic case plan tailor to the individual needs of children and young people living in the ITC-SD residential home and to support them to achieve their case plan goals.

You will be based in the Far North Coast and will hold on to case load span from our services in Grafton, Lismore, Ballina and Central Coast. The children and young people under our care are form trauma background as well as living with significant disability. 

Key Responsibilities  

  • Develop, implement, record and monitor therapeutic case plans for children and young people in LWB's care 
  • Develop and maintain functional and supportive relationships with children and young people as well as their families in order to promote positive outcome
  • Actively engage with clients and family members to gather input for case plans and ensure their views are represented 
  • Coordinate and network with other agencies to effectively achieve case plan goals within tight time frame
  • Participate in a rotational on-call roster for after hours support
  • Ensure compliance with LWB’s policy and procedure, statutory and contractual requirements in relation to management of service delivery 

Skills & Experience 

  • Relevant Bachelor’s Degree in Social Work, Social Welfare, Psychology or similar
  • Experience in managing complex caseloads
  • Extensive understanding in trauma and its effects on children and young people 
  • Experience in working with children and young people in out of home care or similar
  • Experience in working with clients with disability
  • Confident in navigating NDIS and various funding packages
  • Current drivers’ licence, will require regular travel to Grafton, Ballina and Lismore and occasional travel to Central Coast
  • Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check.
