About the role

Join our Sydney Family Based Care team on a 6 month Full-Time Contract where you will develop and implement Case Plans or Individual Plans for children and young people in Out of Home Care. This position will cover from Hurstville through to Penrith with a caseload of up to 14.   

Key Responsibilities 

  • Drive the implementation and achievement of the case plan goals and plans for permanency of children and young people
  • Develop and maintain functional and supportive relationships with children and young people in out of home care as well as their families in order to promote positive outcomes
  • Provide ongoing support and supervision to Foster Carers in relation to their caring for children and young people in their home
  • Support and monitor placements and ensure carers participate in the case planning process and assist them to implement the strategies outlined in the case plan
  • Meet with clients and family members to gather input for case plans and ensure their views are represented
  • Participate in an on call roster as required

Skills & Experience

  • Bachelor degree in Social Work, Social Science, Psychology or Human Services
  • Experience as a Case Manager within the community services field
  • Experience working in adoption, guardianship, restoration and/or kinship care and assessments
  • Strong written and verbal communication and client/stakeholder engagement and mediation skills
  • Sound time management skills with the ability to work under pressure and multitask
  • Current NSW driver's licence
