About the role

Challenging and rewarding opportunity for you to make a positive change in the lives of children and young people in Out of Home Care. 

The case manager sits within the Child, Youth and Families team and will provide placement support to the children/young people and their carers. You will hold a caseload of children and young people and will be required to build a rapport with the child/young person, conduct regular home visits, work with the care team and implement Looking After Children (LAC) requirements.

This role is for a permanent position and will be based in Sunshine. This role will require travel across the metro area and as required to regional areas. There may also be a requirement to work outside business hours.

Key Responsibilities

  • Develop and maintain functional and supportive relationships with children/young people, foster carers, families, and wider care team members in order to promote positive outcomes.
  • Working to KPI’s with a heavy focus on administrative reporting
  • Develop and implement care plans for children and young people in care, including monitoring their health, education, and behaviour needs.  
  • Provide ongoing support and supervision to carers
  • Support and monitor placements ensuring carers participate in case planning process
  • Participate in on-call roster as required

Skills & Experience

  • Degree in Social Work, Social Science, Psychology or Human Services
  • Experience within casework practice for children and young people
  • Experience working with children and young people in out of home care and Foster carers 
  • Experience developing quality case plans or individual plans 
  • Demonstrated experience working autonomously with multiple clients simultaneously
  • Experience working with individuals, families, or groups from a culturally appropriate perspective
  • High-level communication, influencing, liaison, negotiation, and mediation skills
  • Willingness to engage in LWB evidence-based trauma training.
  • Knowledge of the legislative requirements of supporting children and young people in out-of-home care.
  • Ability to maintain positive relationships with individuals, whilst being able to have honest and at times difficult conversations.
  • Current unrestricted driver licence

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
