About the Role

The purpose of the Case Manager – Strong Mob Program position is to develop and implement care/case plans for Aboriginal and Torres Strait Islander peoples with a mental health condition in partnership with the relevant government agency, children and their families, carers and other stakeholders.

The incumbent will work closely with Aboriginal and Torres Strait Islander peoples and communities and will need to demonstrate culture understanding and knowledge of issues impacting Aboriginal and Torres Strait Islander peoples and communities.

Key responsibilities    

  • Provide clear and detailed information to inform the development of case plans, or individual plans.
  • Develop and implement high quality case plans or individual plans for LWB client.
  • Ensure the CIRTS database is up to date with all client information.
  • Support and maintain client welfare and interests at all times.
  • Support and Coordinate the support team involved in the delivery of the service.
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery.
  • Develop and maintain functional and supportive relationships with Aboriginal and Torres Strait Islander peoples with a mental health issue and Family and Carers in order to promote positive outcomes.
  • Develop and maintain relationships with Carers/family members and significant others in order to promote positive outcomes for clients.
  • Manage communication with key stakeholders, internally and externally, including with Aboriginal communities.

Skills and Experience

  • Demonstrated experience to work autonomously in carrying out agreed case plans, or individual plans, for multiple clients simultaneously.
  • Demonstrated experience in managing competing priorities and to deliver high quality outcomes within agreed timeframes.
  • Demonstrated experience in working effectively as part of a multi-disciplinary team.
  • Demonstrated experience working with individuals, families and/or groups from a strength-based, culturally appropriate perspective.
  • Demonstrated experience in working with Aboriginal and Torres Strait Islander peoples with a mental health issue.

Essential Requirements

  • Tertiary qualification in Human Services or related discipline or equivalent relevant experience.
  • High level of knowledge and understanding of Aboriginal and Torres Strait Islander cultures and the issues faced by Aboriginal and Torres Strait Islander peoples and communities.
  • Current Unrestricted Driver Licence.
