About the Role

As a Client Support Officer within our Aged Care and Veterans Client Services team, you will be responsible for undertaking client assessment and reviews, developing, monitoring, and reviewing service provision and goals in order to ensure the individual needs and goals of clients are met. This role is centred around the ACT Community Assistance & Support Program (CASP) - prior experience in this including Case Management is highly regarded.

Our ideal candidate will have a passion for Aged Care with experience in a similar role as a Client Support Officer or Aged Care Case Manager. They will also have a sound understanding of other service offerings including Commonwealth Home Support Program, Home Care Packages and Veterans Home Care.

This is a Permanent Full Time position based in our Griffith ACT office to start ASAP.

Key Responsibilities

  • Undertake client assessments and reviews with a strong case management aspect
  • Produce high quality and comprehensive client support documents
  • Maintain communication with clients in order to support service development, planning and delivery
  • Work with clients to identify and achieve their needs, expectations and goals
  • Maintenance of referrals, client portal records and service bookings
  • Provide case management and/or coordination services for clients
  • Manage and monitor client budgets, funding and expenditure

Skills & Experience

  • Cert III or IV in Case Management, Human Services, Aged Care or other relevant qualification
  • Strong experience in Case Management (or similar role type) is an essential requirement
  • Working knowledge of the ACT Community Assistance & Support Program
  • Sound knowledge of the Commonwealth Home Support Program and Home Care Packages
  • Ability to apply person-centred thinking to all services
  • Ability to build strong relationships and work collaboratively with a range of stakeholders
  • Strong communication, time management & computer skills

Successful candidates will be required to clear probity checks including National Criminal History Record Check and obtain a Working with Vulnerable People Card.
