About the Role

Working in line with the CARE model you will strive to enhance the lives of children and young people in our Child Youth & Family programs incorporating a therapeutic, trauma informed approach. To find out more information on the CARE framework please follow this link.

This is a challenging and rewarding opportunity for you to make a positive change in the lives of children and young people in Out of Home Care. Responsible for the development and implementation of Case or Individual Plans for children and young people as well as providing support to the Foster Carers.

Based at our Maroochydore office, this position is being offered on a permanent full-time basis.

Key Responsibilities

  • Develop and maintain functional and supportive relationships with children and young people in Out of Home Care in order to promote positive outcomes.
  • Develop and maintain relationships with family members, Carers and significant others in order to promote positive outcomes for clients.
  • Meet with clients and family members to gather input for case plans and ensure their views are represented.
  • Support and maintain client welfare and interests at all times.
  • Manage communication with key stakeholders, internally and externally.
  • Develop and implement high-quality case plans or individual plans for LWB clients.
  • Provide guidance, supervision and support to Support Workers.
  • Communicate and act in ways that are consistent with Life Without Barriers Values of Responsive, Imaginative, Courageous, Respectful and Relationships.

Skills & Experience

  • Tertiary qualification in Disability, Mental Health or Community and Social Services or related discipline.
  • Demonstrated understanding of current casework practice for children and young people.
  • Demonstrated experience in working with children and young people in out-of-home care, people with a disability or people with a mental health issue.
  • Experience in developing quality case plans or individual plans.
  • High-level organisation and time management skills, which includes the ability to reorganise work priorities to meet changing demands.
  • Demonstrated ability to work with individuals, families and/or groups from a strength-based culturally appropriate perspective.
  • Demonstrate alignment to LWB values.
  • High-level communication, influencing, liaison, negotiation and mediation skills.
  • Current unrestricted Driver Licence.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
