We currently have a permanent vacancy for a full-time Case Manager to join our Foster Care Team in Parkes.

The People We Support deserve the absolute best Case Managers who are invested in their journey to independence and we're on the look out to find them!

Our Case Managers are responsible for providing case management support to children, young people, their families and carers through a framework of strengths based case management and person-centred approaches.

You will be responsible for:

  • Managing a caseload of children/young people and their families or carers
  • Undertaking comprehensive assessments
  • Initiating, reviewing and completing appropriate intervention strategies
  • Convening and participating in case conferences
  • Facilitating access to services, contributing to and undertaking case reviews
  • Establishing and maintaining a list of contacts
  • Working with external stakeholders and applying understanding of child protection related Court procedures and practices.

If you're able to work collaboratively in a multidisciplinary team and in partnership with other services, and you have experience working with Children and Young People who have experienced complex trauma backgrounds we'd love to hear from you!

The ideal candidate will have the following:

  • Bachelor's Degree in Social Work, Social Welfare, Behavioural Science, Psychology, Social Science, Nursing and Mental Health
  • Experience with Case Management in the Community/Social sector
  • Sound practice and knowledge or significant experience in meeting the holistic needs of Children and Young People
  • Experience managing and supporting Children and Young People who have complex trauma backgrounds and/or who display difficult or challenging behaviours
  • Demonstrated knowledge of the Children and Young Persons (Care and Protection Act) 1998 and Out Of Home Care standards
  • Strong analytical and assessment skills
  • Strong reflective practices and a willingness to accept feedback from stakeholders
  • Drivers license
  • Ability to work flexible hours
