about the role

We are looking for a passionate and caring Case Manager to join our Forster Care team — an individual who can work collaboratively to deliver services through a framework of strength-based case management and person-centred approaches.

As an experienced Case Manager, you will understand the importance of meeting the complex needs of the Children, Young People, and families we support, as well as the responsibilities under the Child Care and Protection Act.

Own It - How we get the job done

The Case Manager – Foster Care will be responsible for:

  • Managing a caseload of children/young people/adults and their families or carers
  • Comprehensive assessments to ensure the development, implementation, and review of individual strength-based case plans
  • Appropriate intervention strategies e.g., risk assessments, referrals and program applications
  • Consultation with all relevant stakeholders working towards identified goals
  • Applying an understanding of child protection related Court procedures and practices
  • Convening and participating in case conferences ensuring inclusion of all relevant parties and the development of a sound strength-based case plan
  • Facilitating access to services and provide comment on their effectiveness
  • Contributing to and undertaking file and case reviews, placement assessment and matching procedures, complex case panels as required
  • Positively embracing diversity and adopting an anti-bias approach
  • Preparing reports and correspondence relating to the implementation and progress of case plans, to inform stakeholders or for inclusion on individuals records.
  • Ensuring activities and reports comply with policy, legal, legislative, funding body and ethical requirements
  • Prepare Court testimonies including providing expert evidence and legal documentation
  • Reporting to Community Services all cases of suspected significant risk of harm to children and/or young people as per mandatory reporter requirements.

Make It Matter - What you will bring to make a difference in other's lives

Essential Requirements:

  • Experience with Case Management in the Community/Social sector (as a Social Worker or Case Worker)
  • Tertiary qualifications in Social Work, Social Science, Behavioural Science, or Psychology
  • Sound knowledge of the Child Care and Protection Act
  • Ability to work collaboratively as a member of a multidisciplinary team and in partnership with other services within the organisation and community
  • Sound practice and knowledge or significant experience in meeting the holistic needs of Children who have experienced significant trauma and/or a disability
  • Most of all to have the passion and drive to help support Children to create positive outcomes in their lives
  • Experience managing and supporting Children and Young People who have complex trauma backgrounds and/or who display difficult or challenging behaviours
  • Strong analytical and assessment skills
  • Strong written communication and record-keeping skills
  • Ability to work flexible hours

Please note: 

The NSW Government Public Health Order requires that all care workers must have their first COVID-19 vaccine dose by 9am 25 October 2021 and two doses by 9am 29 November 2021.  Evidence of a COVID-19 Certificate, or medical contraindication certificate will be required.
