About the role

An opportunity exists for an experienced Community Service Worker who can go above and beyond when it comes to delivering services to partners and children of veterans (also known as Beneficiaries). You will be confident and experienced in working from a strengths based, person-centred approach. You will be known for your excellent interpersonal and relationship-management skills. You will be proactive and are looking to join an organisation that is dedicated to enhancing lives.

This position is part time, 3 days per week (21 hours) and will require you to work from home and the Tamworth Legacy office, with visits to beneficiaries which may involve occasional travel within a 200 km radius (travel expenses reimbursed).

Key Responsibilities

  • Conduct regular home visits to beneficiaries located in the assigned regional area.
  • Advocate for beneficiaries by using a holistic person-centred approach when making referrals or assisting beneficiaries to make referrals for support to maintain their independence.
  • Assist in the development and implementation of case plans (Outcome star) for beneficiaries and review on a 6 month basis.
  • Identify and respond to requests for Personal Response Unit’s for Beneficiaries and refer to appropriate provider (DVA or Legacy). Provide support for installation, decommission and upgrade as required.
  • Assist beneficiaries (and where applicable their carers) to navigate My Aged Care, NDIS pathways or service providers.
  • Provide quality services to Beneficiaries with Disabilities (BWDs) and their families.
  • Provide quality services to young families, which recognises potential financial disadvantage of widows/widowers and optimises educational opportunities for their children.
  • Provide advice to Legatees to supplement the support they provide Beneficiaries, who may have more complex care needs.
  • Develop a sound understanding of key issues for beneficiaries to inform service improvement and policy development.

Skills and Experience

  • Certificate III in community services, welfare or aged care services or above
  • High level of interpersonal/ communication skills and proven ability to engage in and build trust and rapport with internal/external stakeholders
  • Highly developed organisational skills, with attention to detail and the ability to establish priorities and meet deadlines
  • Proven ability to work independently and as part of a team
  • Ability to travel to visit beneficiaries with use of own private motor vehicle
  • Current NSW Class C driver’s license (unrestricted)

Highly Regarded

  • Sound knowledge of government services such as DVA, My Aged Care and NDIS
  • Current First aid certificate or willing to obtain
  • Experience working within the community sector
  • Previous experience with CRM and online case management tools
  • Knowledge of trauma informed practice, person centred practice and a strength based approach to service delivery
  • Experience with Microsoft Office suite of products including Word, Excel, Outlook and Teams
