We are looking for a passionate Team Leader Service Coordination who will lead, develop and support a team of Service Coordinators who source and coordinate the provision of support services to clients across Victoria in the delivery of Commonwealth funded Home Care Packages.

As the Team Leader you will work closely with the Manager and other Team Leaders to ensure that program outputs are delivered in accordance with the HCP Service Delivery Model, accreditation and program standards and relevant legislation, policies and procedures.

The Team Leader will provide subject matter expertise to the service coordination team. They will also be innovative, model a collaborative, consumer-directed care approach and seek actively to empower aged care services clients and deliver excellent customer service.

To be successful, you will require;

  • A relevant tertiary qualification.
  • Current experience in the provision of home-based services to clients of aged care services.
  • Demonstrated experience as a supervisor/team leader, preferably in an aged care service environment.
  • An Employee Working with Children Check will be required for this role (must be obtained prior to commencement).
