LCHS currently has opportunities available within the Integrated Primary Health Services (IPHS) Teams Central and West for registered nurses with drive and passion for providing community nursing, inclusive of generalist and palliative care. These teams have recently undergone a restructure facilitating a true multidisciplinary environment for clinicians to achieve positive outcomes for our clients and their families.

In this role you will conduct assessments, facilitate care planning discussions and complete psychosocial screening for all clients admitted to our IPHS service.

To be successful you will have:

  • Keen interest in palliative care or a willingness to undertake education related to palliative care
  • Current AHPRA registration
  • Minimum of two years nursing experience in community as a Registered Nurse or have successfully completed a graduate year.
