LCHS currently has opportunities available within the Integrated Primary Health Services (IPHS) Teams Central and West for registered nurses with drive and passion for providing community nursing, inclusive of generalist and palliative care. These teams have recently undergone a restructure facilitating a true multidisciplinary environment for clinicians to achieve positive outcomes for our clients and their families.
In this role you will conduct assessments, facilitate care planning discussions and complete psychosocial screening for all clients admitted to our IPHS service.
