As a Linkages Care Coordinator, you will assist people who have fallen through the cracks or aren’t adequately linked into any existing services. You would be responsible for assisting those individuals with a plan, to set goals and develop independent living skills through the use of self-directed planning.

To be successful, you will require:

  • Tertiary level qualifications in the Aged Care industry.
  • Extensive experience in an equivalent of similar role.
  • Thorough understanding and use of the HACC-PYP, CHSP or HCP or NDIS programs in previous lines of work.
  • Current Victorian driver licence.
