Why this role matters

The Initial Assessment and Planning (IAP) program is the main access point for clients experiencing, or at risk of homelessness, throughout Victoria. The IAP program helps people prevent and end homelessness by providing assessment, planning, prioritisation, accommodation assistance, advice and referrals for additional support. The IAP program is part of the Launch Housing Getting Housing portfolio, which consists of three designated Entry Points services at Collingwood, St Kilda and Cheltenham and a small number of Out posted locations. The IAP Worker works as part of a team consisting of Coordinators, other IAP and Prioritisation List workers.

What we're looking for

We are currently seeking a skilled & passionate individual, someone committed to working with people who are experiencing homelessness, or at risk of homelessness on their journey out of homelessness.

It is expected that the successful candidate will bring:

  • To provide initial assessment of housing and support need, information and assistance to clients and to other regional services within the Opening Doors Framework
  • Provide client-centred services to persons experiencing or at risk of homelessness, including crisis intervention, assessment, prioritisation, assistance to secure crisis accommodation, assistance to secure or maintain long term accommodation, distribution of financial assistance and linkages to appropriate support services
  • To work as part of a multi-site team delivering face-to-face and telephone services in a high-volume service
  • Utilise assessment tools to a high level of fidelity in the identification and prioritisation of people in the most urgent need of program resources
  • Maintain client files and records in accordance with procedures on a computer case management software system
  • Advocate on behalf of those experiencing homelessness to ensure that services are relevant, flexible and accessible to people experiencing homelessness
  • Ensure clients are assisted to understand their rights & responsibilities in the provision of accommodation and support assistance
  • Ensure services are informed by client participation and ongoing feedback
  • Comply with all risk, health & safety and compliance policies, frameworks and procedures
  • Ensure that all accident, incident, risks and hazards are identified and reported in own work context
  • Ensure safety of self and others in work environment taking reasonable care that actions or omissions do not impact on the health and safety of others
  • Being familiar and aware of relevant legislation, emergency and evacuation procedures and ensures compliance with all relevant work practices
