Why this role matters

The Continuing Care Worker will provide ongoing oversight, support and referrals to clients from the Enhanced Engagement Program living in the community. The role will use active engagement skills to develop rapport with clients whilst they are in crisis accommodation, with a view to having an established relationship once the client moves into the community. The worker will continue to support the clients once housed, to support them to build connections within the local community.

What we're looking for

We are currently seeking a skilled & passionate individual, someone committed to working with people who have previously experienced homelessness, engaging with them to support their independence and their journey within stable permanent housing.

Key responsibilities

  • Support clients living in stable housing when exiting from enhanced engagement.
  • Continue working towards case plan goals as identified by clients.
  • Engage with clients whilst in crisis accommodation to build rapport and provide a smooth transition from enhanced engagement to continuing care.
  • Engage other supports when client presents with or identifies issues requiring more support, with a view to sustaining stable housing.
  • Have regular contact with clients from the enhanced engagement program over the two years of the support period.
  • Develop existing and new relationships and/or partnerships that will enable coordinated service delivery.
  • Leverage partnerships (where appropriate) to expand and improve services and client outcomes.
  • Embed effective collaboration and communication with internal and external stakeholders.
  • Comply with all record keeping, reporting and evaluation processes as applicable and directed.
  • Work collaboratively with external agencies to develop a coordinated model of support specific to addressing the needs of each individual within this cohort.
