Why work in the H2H (Homelessness to a Home) Program and who we are looking for?

H2H is a once in a generation initiative that aims to provide sustainable long-term housing for those with a prolonged history of rough sleeping and homelessness. Created in response to the thousands of Victorians who had nowhere safe to stay when the Covid-19 Pandemic struck, the program aims to successfully transition clients out of emergency accommodation options into a home where they feel safe and secure.

We are currently seeking experienced Outreach Case Managers to join our well-established team. You will play a significant role in assisting people experiencing homelessness and housing insecurity across Melbourne.

The ultimate objective is to assist the client group into safe, sustainable and affordable long-term housing options, providing support every step of the way. This role represents a chance to work within a pioneering program, one that promises to utilise the unique opportunity that the COVID-19 pandemic has created, in supporting previously hard to reach individuals.

You will engage with diverse and complex individuals to provide wellbeing focussed case management support and deliver innovative and effective services to our clients. You will build relationships and advocate with specialist support services to improve the client's health and wellbeing. This is an exciting opportunity to join a reflective and encouraging team supporting and empowering people to develop the skills, capacity and linkages to ultimately improve their quality of life and avoid future episodes of homelessness.

Successful applicants will also have:

  • A tertiary qualification in human services, social work or other relevant discipline
  • Demonstrated understanding of housing and homelessness policy, context and systems
  • Demonstrated ability to work collaboratively with others towards effective client solutions
  • Experience working with people with complex needs
