Case Manager - Community Residential Facility Transition Support

Jesuit Social Services
  • Exciting new pilot program
  • Maribyrnong location

About the organisation

Jesuit Social Services is a social change organisation working to build a just society where all people can live to their full potential.

Since 1977, we’ve provided services in some of the most disadvantaged in our community.

We place a high priority on advocacy and are a leader in policy development and research.

About the program

Our Justice programs provide support for people involved in the justice system or who are exiting adult prisons and/or Youth Justice Precincts, assessed as high risk/need, with limited social and family networks, limited accommodation and post-release support options and often with multiple and complex health concerns.

Our approach to working with those involved in the criminal justice system uses empowerment as a key practice principle, with a focus on skills and strengths rather than deficits, and self-determination and responsibility rather than dependency.

The Community Residential Facility (CRF) Transition Support program is looking for an experienced Case Manager to join our team.

The CRF will provide an accommodation option for men assessed as suitable for a residential facility, with a strong focus on risk and community safety. The facility will provide accommodation for post-release offenders and parole release offenders.

To be eligible, men will need to have no immediate access to suitable housing on release from prison (no other available options) and need accommodation support to comply with the Government’s social distancing requirements under the Chief Health Officer’s directions. It is proposed that all serious sex offenders and high risk serious violent offenders be excluded from eligibility for the facility.

This exciting new Pilot program is a partnership between Corrections Victoria and Jesuit Social Services. Jesuit Social Services will deliver pre and post-release case management and transition support to men at the facility, including access to longer term housing, assistance to attend health services, access to learning and vocational opportunities and links to cultural support. The focus of the work is to support men successfully transition to independent living in their communities of choice.

About the role

We seek a qualified, experienced and enthusiastic individual to deliver case management and outreach support to men on the program. The role will work across our Brunswick site and the Maribyrnong residential facility, with outreach services delivered across metropolitan Melbourne.

The successful applicant will be responsible for providing individualised case management and transitional support to men as they transition and reintegrate back into the community.

A base salary of $72,361 plus superannuation and salary packaging is offered, together with ongoing professional and career development.

Candidate Requirements

The role will suit candidates with the following skills and experience:

  • Tertiary qualification/s and/or experience in field required
  • An understanding and empathy for the challenges facing men as they transition from custody to the community.
  • Experience in the delivery of case management and outreach support services.
  • Experience working with men to address the underlying issues that contribute to offending behaviours and a knowledge of specialist services and community based support/s.
