ABOUT YOUR ORGANISATION

The Junction Neighbourhood Centre (JNC) has been supporting people in Eastern and Inner Sydney for over 50 years. JNC provides a diverse range of services and programs for the communities in Eastern and Inner Sydney, with a focus on people who are socially isolated, disadvantaged and vulnerable. JNC works with older people, families, people experiencing Domestic and Family Violence as well as the broader community. The focus of JNC’s work is strengthening local communities and creating connections. We have community centres in Maroubra, Glebe and Bondi Junction which provide community information and places for the community to meet and participate in a wide range of groups, classes and programs.

ABOUT THE ROLE

As an Aged Services Coordinator you’ll support older people accessing support through the Commonwealth Home Support Program. You will assess needs, create care plans and service plans and roster services on a weekly basis. You will work closely with Social Support Workers and Volunteers to ensure we provide person centred and inclusive services. You will promote our services at local expos and community events.

Key responsibilities

  • Support clients under the Commonwealth Home Support Program (CHSP) by completing assessments and personalised care plans
  • Visit clients in their homes to undertake risk assessments
  • Develop, maintain and review rosters of field staff and client service schedules
  • Respond to service related questions or issues with Social Support Workers
  • Respond to incidents in service delivery and client or carer feedback and complaints
  • Update the client database with care plans, service agreements, updates and case management notes
  • Track the progress of our programs against contracted outputs and targets
  • Ensure that data is available for the timely completion of internal and external reports
  • Complete weekly financial tracking sheets

skills and experience

  • Relevant qualifications in social work, aged care or social services and/ or at least 2 years’ experience in community aged care
  • Knowledge of community aged care and health service systems, intake and assessment, case management and coordination
  • Knowledge and understanding of Aged Care reforms
  • Demonstrated abilities to design and prepare care plans, implement consumer-directed services across a diverse client base of older people (including CALD, LGBTIQ++ and First Nations clients) and build rosters for support workers
  • Well-developed communication and interpersonal skills with a compassionate approach to working with older clients
  • Understanding of wellness approaches in planning and delivering services for older people
  • Awareness and sensitivity of cultural diversity in service delivery
  • Experience in supervising and supporting staff
