This locum parental leave role as an Aged Services Case Manager will give you the exposure and experience for a future leadership role. In this part time, job share role, you will provide oversight of the quality of social support services to ensure continuous improvement, innovation and development required to meet client needs and goals. You will provide team leadership and individual support to Social Support Workers. You will bring a person-centred approach in working directly with clients (and families or carers) to set individual goals and develop tailored social support and respite services to enable them to live at home safely.

To be successful in this role, you will have knowledge of community aged care services and a strong knowledge of community care sector, intake and assessment, case management, and care coordination. You will demonstrate your abilities to design and prepare care plans and implement consumer-directed services across a diverse client base of older.
