What we do

Jewish Care Victoria, a not-for-profit organisation, is the largest provider of aged and community services for the Jewish Community in Victoria. Services include Disability, Mental Health, Youth and Family Services, Aged Residential Care, Assessment Referral and Individual Support Services, Community Aged Care and Holocaust Survivors Programs.

The role

As a Support Coordinator you will provide in-home supports to help Elders live comfortably in their own homes.

You will have a qualification in a human services field such as; Nursing, Social Work, Allied Health or similar and use those skills to meet the requirements of a busy caseload of clients with their needs. Some experience in coordinating a caseload of clients is essential! You will be computer literate, detail oriented, able to multi-task and problem solve and genuinely enjoy meeting Elders from a variety of cultures and hearing their stories.

What you will do

  • Identify needs based on an assessment of the Elders’ functional capacity, via discussion with them, their family or support person.
  • Visit Elders in their homes in order to undertake assessments reviews and maintain contact with them and their families.
  • Develop Support Plans in collaboration with Elders that meet their goals.
  • Develop and maintain a budget in collaboration with your client in accordance with the Australian Government Guidance for Home Care Packages.
  • Apply a broad knowledge of resources (or know where to locate such resources) in response to identified needs.
  • Use of interpreters either by phone or in person when required.
  • Manage a busy schedule of home visits, reviews, emails and phone calls.
  • Fulfil reporting and compliance requirements within required timeframes.

What you will bring

  • Understanding of the health and welfare issues faced by older Australians wishing to remain in their homes.
  • Experience in undertaking complex in home assessments, implementing Support Plans and managing budgets.
  • Evidence of fluent written and oral communication skills.
  • Demonstrate compassion and empathy when working with Elders, including ethno-specific groups and Elders affected by trauma such as Holocaust Survivors.
  • Demonstrate technological competency skills necessary to perform job skills (i.e. Microsoft Office suite and client management systems)
  • Tertiary Qualifications in Human Services, Nursing, Allied Health, Health Management or similar.
  • Ability to work autonomously, prioritise and multitask.
  • Ability to speak Russian will be well regarded but not essential.
  • Case Management experience is desirable.
