The Institute for Urban Indigenous Health (IUIH) leads the development, planning and deliver of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.

IUIH Connect Plus connects care for Aboriginal and Torres Strait Islander people transitioning from hospital to primary healthcare and into the community. The IUIH Connect Plus team is committed to working together with all providers and stakeholders to provide culturally safe quality care for clients and their families.

About the Role?

As a Paediatric Integrated System Coordinator with the Connect Plus you will work collaboratively with the Children’s Hospital Queensland (CHQ) and IUIH to develop integrated service delivery models across primary and tertiary healthcare sectors.

  • Co-located at Windsor and CHQ
  • Oversee the partnership with CHQ, within the formal agreement between IUIH and CHQ to provide Aboriginal and Torres Strait Islander clients access to regional services is SEQ
  • Support the development and drafting of policies and procedures whilst working alongside iCare team
  • Aiding service managers in the smooth integration of services between IUIH and CHQ by engaging in in-services to teams within the CHQ network
  • Establish new ways as well as working in accordance with current Project Plan targets to disseminate/share information with clients
  • Identify current channels and support the integration of IUIH Ways through client and CHQ relationship
  • Oversee and ensure that 100% of children are provided with information on the IUIH Connect Plus Program

About You?

Essentials:

  • Current AHPRA Nursing Registration or Allied Health Professional
  • Valid Blue Card (QLD Working with Children Check)
  • Valid Disability Worker Screening Check
  • Current First Aid Certificate
  • Current QLD Drivers Licence
  • Current Criminal History Check
  • Ability to travel across SEQ as required
  • Must have at least Dose 1 of the Covid-19 vaccine prior to commencement

Desirable, but not mandatory:

  • Previous experience working with Aboriginal and Torres Strait Islander people and /or communities
