About Us?

The Institute for Urban Indigenous Health (IUIH) leads the development, planning and deliver of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.

IUIH Connect Plus connects care for Aboriginal and Torres Strait Islander people transitioning from hospital to primary healthcare and into the community. The IUIH Connect Plus team is committed to working together with all providers and stakeholders to provide culturally safe quality care for clients and their families.

About the Role?

As a Care Coordinator for the IUIH Connect Plus team your aim will be to reduce the significant points of risk in a person’s journey through the health care system, in particular, at hospital entry and discharge, and in transit from primary care in and out of specialist, allied health and other services.

We currently have two positions available:

  • 1x Permanent, Full Time P.M shift (Monday to Friday 11:00am to 7pm) located at Darra
  • 1x Permanent, Full Time standard hours located at Darra

  • Being the main point of contact and access for clients, which may be directed from a number of different client referral pathways
  • Assessment of client needs in consultation with the client and work with the client to develop an action plan that sets out priorities and actions - assessing needs, planning for discharge, home visits, case conferencing and advocacy
  • Liaising with other service providers and forming a bridge from tertiary to primary healthcare
  • Work collaboratively internally as well as externally to achieve best results for clients through networking, sustaining partnerships and organising in-service programs
  • Provide regular updates to the IUIH Connect Manager including data, statistics, referrals  received and identified barriers to client access and engagement

About You?

Essentials:

  • Minimum qualification of Diploma of Nursing with a preference of Bachelor of Nursing
  • Current AHPRA registration
  • Current C Class Drivers Licence (QLD)
  • Current First Aid and CPR Certificate
  • Ability to communicate, engage effectively and work together with Aboriginal and Torres Strait Islander peoples.
  • Must have at least Dose 1 of the Covid-19 vaccine prior to commencement

Desirable, but not mandatory:

  • Previous experience working with Aboriginal and Torres Strait Islander people and /or communities
