about the role

The GP Practice at South Melbourne has been operating since 2013. It is a busy service, with a diverse client group. Most of our clients are bulk billed and have complex social and health needs.

The practice is closely linked to other community services in the area including Launch Housing (crisis accommodation service), St Kilda Road Clinic (community mental health service), Access Health and First Steps (AOD service). Star Health also has a range of internal services and programs for our clients, including, dental services, Allied Health, AOD, Mental Health and other community-based programs.

As a Clinical Care Coordinator, you will be responsible for working with patients to identify their goals, support them with chronic disease self-management, and coordinate services and providers in order to meet those goals.

Ideally, you will possess

  • A professional qualification in Nursing (Division 1 RN)
  • Experience in a community health or primary health care setting, working with complex clients
  • Demonstrated ability to work independently and meet program KPIs
  • Excellent knowledge of coordination of health, community and social services to meet a patient's needs
  • A clear understanding of developing care plans and health assessments for clients with complex health and social issues such as: mental health, AOD, people from CALD backgrounds, Aboriginal and Torres Strait Islander peoples.
