  • Base Salary + Super + salary packaging (up to $15,900 tax free wage)

The Role:

Reporting to the Disability Supports Manager, you will provide coordination and management of tasks working closely with under supervision of Disability Supports Coordinator. You will be managing and maintaining new client enquires and linking them with appropriate Disability Supports Coordinator and attend home visit to communicate clients on changes/considerations required with individualised service plans. You will actively communicate with all internal and external stakeholders to pursue comprehensively appropriate care for each Disability Supports client. This position will be based at Wharf Rd, Newcastle.

Job tasks and Responsibilities:

  • Assisting the Disability Support Coordinator by providing detailed case notes and feedback received
  • Active management of issues/concerns and changes related to client centred services
  • Maintain clear organisational communication processes to provide transparency and timely attention to changes in service delivery requirements and caseloads
  • Responds to requests for additional services/changes in a timely manner ensuring information is communicated clearly and effectively
  • Maintain current knowledge on changes to programs and funding related to client service delivery
  • Participation in meetings and application of new ideas and processes related to client service
  • Participate in review and evaluation process and adopt continuous improvement focus

Skills and experience:

  • Combination of tertiary qualifications in a relevant field and knowledge/experience gained in a similar role in Disability Services
  • Knowledge of the available government funding structure for Disability supports services
  • Understanding of NDIS and service delivery requirements
  • Self-directed and accountable for own actions
  • Excellent communication skills
  • A compassionate and caring disposition with a strong personal work ethics
  • A strong desire to provide excellent service to disability Clients
  • Ability to foster our culture which is based on respect and dignity for our clients
