About Role

Case Managers operate within the community to work with our clients, their families and the wider care network, allowing elderly Australians in regional and remote communities to access services of their choice. They help achieve creative and focused services through responsive and considered case management. Acting as a trusted advisor case managers positively affect their decision-making to achieve high client satisfaction.

Case Managers conduct person-centred assessments to develop our clients' care plans and budgets to provide fulfilling care and support by facilitating their choices. And, working collaboratively with other health professionals in the organisation and externally, assess changing customer care needs ensuring the best care is delivered.

Responsibilities

  • Provision of high-quality, values-based case management for a diverse caseload
  • Conduct person-centred assessments that result in strength-based goals and outcome measurement
  • Develop networks and partnerships with important partners
  • Manage individual budgets, achieve required program outputs and report through to the Senior Case Manager
  • Promote services and identify opportunities to expand our portfolio of services
  • Maintain safe practices through risk management strategies

Skills you need to succeed

  • Combination of tertiary qualifications in a relevant field and experience of 2 or more years gained in a similar role in disability, aged care, youth work
  • Customer service experience in community engagement and ability to further grow the footprint
  • Experience to work remotely or as part of a team, with the ability to guide and motivate staff to provide safe, quality, person-centred services
  • Current Drivers Licence
  • Ability to travel within Northern Tasmania and East Coast
