The Role

Seeking a dynamic and experienced NDIS Support Coordinator / Specialist Support Coordinator to join our team in Victoria.

Reporting to the Psychology and Support Lead, this role provides an exciting opportunity to build capacity across the sector, influence outcomes and facilitate empowerment within a collaborative team environment.

The role involves supporting NDIS funded Participants to:

  • Identify and work towards specific goals in a complex, intensive and highly adaptive setting
  • Navigate within hospital to home, rehabilitation, YPIRAC, housing and life transition spaces
  • Connect and engage with relevant NDIS funded services and support networks
  • Access and identify mainstream and community supports across multiple interfaces
  • Explore and maximise available resources and funding with a holistic approach
  • Communicate and collaborate towards measuring and reporting on outcomes

The role involves building and sustaining strong corporative relationships with government and non-governmental bodies and establishing professional relationships with service providers.

What you will bring to the role

  • An Allied Health degree is highly desirable. Minimum Certificate IV in Community Services or equivalent demonstrated skill level if working towards a higher qualification
  • Minimum 2 years’ experience working as a Support Coordinator, LAC, or NDIA Planner
  • Comprehensive working knowledge of the NDIS framework, funding and practical experience in the implementation and review of NDIS plans, relevant Legislation and guidelines
  • Extensive experience working within the hospital to home setting, housing space and / or life stage transition space, focusing on independent living choices, rehabilitation, capacity building and empowerment across multiple interfaces
  • Outcomes focused with a flexible approach to a varied and complex caseload
  • Demonstrated experience effectively prioritising tasks with conflicting priorities
  • Ability to problem solve, think creatively, build extensive networks and expand resources to exceed Participant expectations
  • Proficient with use of Microsoft suite, NDIS Portal and client management systems
  • Capacity to identify barriers, understand communication needs, adapt to various environments and collaborate for optimal outcomes
  • Passionate and focused on providing opportunities for building effective relationships, enhance Participant choice and control and empower self-direction
  • Current and valid Driver’s licence
  • Current Fit2Work Police Check and Employee Working With Children Check (WWCC)
  • Completion of NDIS Q&S Worker Orientation Module
