About the Role

The Outreach Worker will provide assertive outreach supports to engage with people who are experiencing homelessness, who may also have a psychosocial disability, with the objective of supporting them through the NDIS application process. This role is for a fixed term commencing January 2021 and ending in June 2021, remunerated at $31.02 per hour plus optional salary packaging.

Responsibilities include:

  • Undertaking assertive outreach activities to engage with people experiencing homelessness and promoting understanding of the NDIS and relevance to them.
  • Supporting consumers to participate in preliminary assessments to determine potential eligibility for the NDIS.
  • Supporting consumers through their NDIS application.
  • Supporting consumers to identify goals using motivational interviewing techniques.
  • Supporting consumers and service providers to write, gather and collate effective supporting evidence/documents.
  • Supporting the facilitation, set up, pack down and consumer engagement during project events.
  • Assisting consumers and Embark II team to book and attend meetings/appointments with service providers and mainstream supports and to gather supporting evidence forms.
  • Assisting in linking consumers with relevant service providers as needed.

About You

Essential: Experience, Qualifications and Personal Attributes

  • Experience working with homelessness and/or people who live with a psychosocial disability
  • Experience in a wide range of community events and functions
  • Experience working with the NDIS
  • Commitment to trauma informed and consumer led practice
  • Ability to travel across Sydney and within NSW if required
  • First aid, current NSW driver’s license, WWCC, police check and category A vaccinations are all required for the role

Desirable:

  • Certificate 4 in mental health or working towards a tertiary qualification in mental health or disability;
  • Mental Health First Aid certificate
  • Experience with outreach
