An exciting new opportunity exists to join the Youth Engagement & Pathway Unit at Hume. We are looking for a Middle Years & Transition Officer who has experience working with middle years children, families, and schools; is value driven, motivated and has a good understanding of the service sector.
The role sits in the Mental Health, Wellbeing and Leadership Team and is a key point of contact for support and referral pathways for families; and works in partnership with primary and secondary school leaders in the municipality to ensure that middle year's children and their families are supported to transition effectively from primary to secondary school, engage with school and their community and have enhanced mental health and wellbeing outcomes.
