  • Reporting to: Neighbourhood Centre Manager.
  • Salary: Social, Community, Home Care & Disability Services Industry Award 2010, Level 5 to 6, depending on qualifications and experience.

HUB is a not-for-profit organisation that advocates for and engages with the diverse community of Inala and the southwestern suburbs of Greater Brisbane. We offer legal services and community-centred neighbourhood activities that focus on where we can have the greatest impact. We work in flexible ways across Inala, providing a blend of supports through our neighbourhood centre and legal services.

ABOUT THE POSITION AND PROGRAM

The Social Prescribing Link Worker role is a new position embedded in the HUB Neighbourhood Centre Team. The successful candidate will be passionate about the potential of social prescribing in improving wellbeing outcomes for families. The purpose of the program is to support families to enhance their wellbeing by strengthening connections with communities, social networks, and services. The function is ultimately designed to engage families who are at heightened risk of social isolation and loneliness, and address these concerns within community, as a prevention/early intervention approach to broader negative health, wellbeing and social concerns.

POSITION RESPONSIBILITIES

  • Direct Client Support – work with families to assist them to meet their goals.
  • Referrals and Partnerships – establish and maintain relationships with community service providers.
  • Community Development – work collaboratively to improve community support infrastructure.
  • Program Design and Evaluation – contribute to co-design process and play a key role in developing the program.
  • Professional Development – undertake training and supervision and maintain appropriate professional qualifications.

SELECTION CRITERIA - ESSENTIAL

Essential Qualifications, Skills, & Experience :

  • A degree in social work, human services, or similar degree/qualification and/or significant experience in equivalent work.
  • Possess a valid driver’s license and have access to a comprehensively insured vehicle, OR demonstrated ability to travel efficiently by other means between varied work locations for community development activities and partnership building.
  • Eligible to obtain or have a current Working with Children Check (Bluecard).
  • First Aid, CPR and Mental Health First Aid or willingness to obtain.

SELECTION CRITERIA - DESIRABLE

Desirable Qualifications, Experience, and Attributes:

  • Knowledge of local community networks and services, as well as understanding of social prescribing, motivational and behavioural change techniques with a demonstrated ability to problem-solve and overcome barriers to client engagement.
  • Strong assessment and consultation skills with families and children, and experience in providing structured, empowering support to improve health and wellbeing outcomes - including a demonstrated understanding of working child safe manner.
  • Experience in working in a culturally sensitive way with diverse communities, including Aboriginal and/or Torres Strait Islander peoples, and those from culturally and linguistically diverse (CALD) backgrounds.
  • Ability to manage and prioritise workload and client demand in line with community needs, individual support requirements and within scope of a social prescribing model.
  • Strong organisational and administrative skills, including experience managing and recording confidential information, taking comprehensive case notes and contributing to data collection and reporting processes.
  • Strong interpersonal and communication skills, with the ability to confidently engage with a wide variety of stakeholders and build relationships with community service partners for the delivery of the program.
  • Demonstrated experience contributing to program/project design, with a flexible and proactive approach to program/project delivery.
