ABOUT THE ROLE

We are seeking a dedicated and supportive Case Manager with demonstrated experience in helping young people identify their goals and how to transition out of the homelessness service system.

The Hope Street in Whittlesea program is based on the Foyer model of supported transitional housing for young people and young families who are provided with safe, secure and affordable accommodation for 12 months to 2 years.

In this role you will:

  • proactively assist young people to develop their skills and abilities to live interdependently in their local community including enjoying meaningful and sustainable relationships; and
  • provide strength-based case management support to develop interdependent, sustainable life to access and/or maintain employment, education and training opportunities and achieve meaningful citizen participation.

ABOUT YOU

You will have:

  • A bachelor degree in Social Work, Youth Work, Social Sciences or relevant Health Sciences degree.
  • 2+ years Demonstrated experience and knowledge to respond to young people with needs in Transitional Housing or other housing/accommodation settings including knowledge of accommodation and housing referral and intake processes.
  • 2+ years Demonstrated Experience in Case Management, Child, Youth and Family Services.
  • 2+ years Experience of current theoretical approaches and frameworks for working with highly vulnerable young people and their families, including strengths-based approaches.
  • Strong ability to engage, motivate and support young people with complex needs within a case management framework and client centred strengths base, trauma informed practice.
  • Great relationship building skills which enables you to motivate young clients to achieve meaningful goals within a case management framework.
  • Friendly and cooperative nature that helps build relationships quickly with key stakeholders in the community.
  • Highly developed communication skills including report writing, listening, negotiation, problem solving, decision making and advocacy.
  • Strong competency using Microsoft and Case management applications.
